The 2021 Business Impact Assistance Program is open to the following:

• For profit brick and mortar businesses
• Nonprofit organizations within the City of Fountain
• Home-based businesses (network/direct marketing businesses not eligible)
• Mobile/trailer businesses
• Commercial landlords
• Professionals that rent "booth" space from an established business location

To apply you must fill out the application below that is applicable to your business, and submit all required documentation (listed within application).  Incomplete applications can not be evaluated.
